Before you complete this form please read information about the different sheltered housing schemes in Croydon. This is available on our website and as a leaflet which you can download.

On this form you will be asked to select the schemes you are interested in. There is no limit to the number of schemes you can choose, but you must choose at least one. You will also be asked to tell us which four schemes you would most like to live in.
